Mr L Chirosca
was pushing a 3-wheeled container at a meat factory. In the process, one of the wheels became locked, and in attempting to bring it upright he suffered a back injury.

was opening the curtains of a lorry trailer. In the process, a wooden side slat that had not been secured properly fell down, striking him on the left thumb.

was attaching a bag of sugar to an improvised metal framework, on the forks of a pallet truck. In the process, the framework fell onto his right ankle.

was trying to move a portable meat processing machine weighing 2.2 tonnes. In the process, he lost his grip on the lever, which sprang up injured his hand.

was standing on a scaffolding tower, while removing insulation from a ceiling. The scaffolding tower fell over, causing him to suffer 3 fractures of his left arm.

was cutting wood with an electric saw. No workbench had been provided, so he was using the saw single-handed. As a result he suffered a laceration to his right thigh.

was walking inside a warehouse at the start of her shift. A pallet truck drove excessively close beside her, and one of the wheels ran over her foot.
