Mr F Vlad
was emptying a metal cage at a recycling centre in Northampton. In the process he slipped on a discarded plastic bottle and suffered a broken arm.

was using a portable toilet on a building site. Without warning, a forklift picked up the toilet, with the client still inside, dropping it 35 metres away.

was working in a furniture factory. A co-worker rolled his job onto the client’s bench, causing his finger to become compressed against a claw hammer.

was visiting a garage in Swindon. It had been snowing, but no grit had been applied. As a result, he slipped and suffered a broken leg.

was clearing dead leaves from a drainage gully on a cowshed roof. In the process, the roof gave way, causing him to plunge 4 metres to the floor.

was feeding the hoops of a polytunnel into an unguarded machine, on a farm in Staffordshire. In the process his left little finger was dragged in.

was was bending a steel rod by hitting it with a sledgehammer. In the process, the rod bounced back, breaking 2 bones in the client’s left hand.
